Information Letter and Consent Form
Introduction and Description
Welcome to the Past Life survey. You are invited to take part in a survey conducted by Dr. Claire White, California State University, Northridge. The purpose of this research project is to learn more about past life experiences. Specifically, we are interested in why people think they have had a past life. If you believe you have lived previously, then please fill out this short 15 minute survey.
In order to participate in this survey, you must:
1.) Hold the belief that you have lived previously
2.) Reside in either the U.S. or U.K.
3.) Be at least 18 years old
4.) Have a memory of a past life
5.) Be most convinced of your past life by a memory
6.) Not have participated in our first survey
Voluntary Participation and Release and Use
Although your participation in this survey is greatly valued, you have the right to withdraw your consent and leave the study at anytime without prejudice. The results of this study will be used for scholarly purposes only. Additionally, the results of this survey will be published. A link to the publication should appear at: www.clairejwhite.co.uk.
Risks and Discomforts
The only perceived risk associated with the procedures described in this study is that answering questions about one’s past life could possibly elicit negative emotions. There are no known harms or discomforts associated with this study beyond those encountered in normal daily life. Low cost counseling services can be accessed at your own cost through the following programs: CSUN’s Psychology Department’s Anxiety and Mood Disorders Clinic: (818) 677-5085 and Pepperdine University’s Community Counseling Clinics: West LA (310) 568-5752, Encino (818) 501-1678, Irvine (949)223-2570. Your response will remain completely anonymous and will not be traceable to you. So, please feel free to answer honestly. Data will be stored on a password protected computer accessible only by Dr. White and the student research coordinator. Once the online data is collected, the online survey will be deleted.
